Thomas (Dylan O’Brien), seine Freunde und die Widerstandskämpfer von The Right Arm haben sich der mächtigen Organisation WCKD gestellt und wissen nun, warum die „Auserwählten“ so hartnäckig verfolgt werden – in ihnen steckt der Schlüssel zu einem Heilmittel für ein Virus, das „der Brand“ genannt wird und dem der Großteil der Menschheit bereits zum Opfer gefallen ist. Nach einem Kampf sind einige von Thomas‘ Freunden in die Hände der Organisation und deren skrupellosen Leiters Janson (Aidan Gillen) gefallen – doch Thomas kann nicht akzeptieren, dass sie zum Wohle anderer ihr Leben lassen sollen, und auch mit der zur Gegenseite gewechselten Teresa (Kaya Scodelario) hat er noch eine Rechnung offen. Mit seinen verblieben Mitstreitern macht sich Thomas auf den Weg in die Letzte Stadt, wo WCKD seinen Hauptsitz hat – und dieser Weg führt sie ausgerechnet dorthin, wo alles begann.
